I received a letter notifying me of a security breach at Henry Ford Health System concerning my medical records on file. Described were the content stolen, including name, address, phone, date of birth, medical record number, email address, ethnicity, religion, father/mother names, dates of doctor appointments, hospital admissions, and Social Security number. The letter explains that this affected those serviced between 2013-2016. The letter is dated July 14, 2017 regarding the breach noted on May 18, 2017.
Expressed in the letter included an apology and steps being taken, including an investigation by federal and local authorities. Also, the letter indicates that the person who is in the possession of stolen health information is expected to be arrested soon and face criminal charges; and that the person never worked at Henry Ford.
Finally, as a result of the investigation and in recompense for this security failure, Henry Ford Health is offering five year free identity monitoring and protection service #PC-567 through Experian Protect ID Alert by dialing 877-288-8057. Also, by dialing 800-869-5704 you may also request a new medical record number.

I contacted Henry Ford and they are not aware of any breach, although they do occur periodically.
