I was offered $100 gift card from Walmart to take survey took the survey that never ended and out of curiosity the next day I had went back and looked at the page and at the bottom of it I clicked contributors and I was just going to move on but then I noticed that it had my name and information and had me logged into an account at Quality Health. Com I never said that anybody could make an account I never even new about the account just by chance that I found it I’ve tried to contact them twice to have it removed they refuse.
