A consumer responded to an email which claimed that he can receive a $500 Walmart gift card by completing a survey. He completed the survey that included questions regarding insurance, disability, medicare. It was also required for consumer to provide their phone number and email address. Then about a few hours later he received a call from a female that said thanks for the survey, and stated that she was from a company (unknown). She proceeded offer him assistance on obtaining a medicare part B dental/vision plan from all major insurers at little to no cost. She then provided a phone number (855)641-3051 for the consumer to call and speak to a man named Kato James, who will provide him info on receiving a Walmart Gift Card. The consumer called the number several times and no one would answer.
